Scheduling an Appointment

In order to schedule an appointment with Dr. Acunzo, please contact her via email or phone to determine appropriateness of referral and secure your payment and insurance options. If after speaking to Dr. Acunzo or her staff, and an appointment is recommended, you will be directed to complete the intake forms and print the signature authorization:

Prior to your appointment, we expect to have the following:

• A completed intake form and a signed Consent
• A completed history form
• A deposit of $100 toward the coinsurance, deductible, and/or academic achievement testing.  For the balance, credit card, checks, or cash is accepted
• Your insurance card
• You may email forms back, and also print those that need to be signed. Please mail with the deposit to 68 Tupper Road Suite 8 Sandwich, MA 02563

Typically, individuals are tested for approximately 3 to 6 hours, and often in one to three sessions. School meetings are often not covered by insurance or covered by assessment fee.  The fee for such meetings will be required prior to the meeting.
